Greater Trochanteric Pain Syndrome, commonly known as trochanteric or hip bursitis:
- Common in women aged 40-50yrs
- Increase in physical activity levels typically triggers symptoms
- Width & shape of female pelvis predisposes lateral hip to increased loads
- Link with hormonal changes is also being studied further
What is GTPS, and why not bursitis?
Research is now showing ‘true’ bursitis is a relatively uncommon finding when looking at patients with lateral hip pain. Long et all (2013) looked at 877 patients, and only 20% displayed bursitis. 50% demonstrated gluteal tendinosis, a small number had tendon tears and 28.5% had a thickened iliotibial band. Multiple studies have now shown tendinosis essentially occurs concurrently with bursitis. As physiotherapists it is imperative we treat the tendon.
State of the research:
The LEAP trial (2018) undertaken in Australia demonstrated that education and exercise was superior to injection and ‘wait and see’ in managing gluteal tendinopathy. This was a prescribed program administered by physio and progressed over a gradually over time. Cortisone may still have a role to play, but more as an adjunct to ‘commence’ rehab as opposed to a stand-alone intervention.
What we do?
As discussed in last month’s newsletter about tendinopathy, the key is accurate diagnosis, patient education & load management. Our role as evidence-based practitioners, is to develop a plan to manage the condition effectively, rather than a temporary fix of GTPS.
For example:
- Accurate diagnosis and assess contributing factors
- Prescription of specific exercises for hip muscles and kinetic chain
- Advice on footwear
- Walking program – when to take rest days
